FMCSA Authority Revocation Protection
If for any reason, the FMCSA issues a notice against your company, you have a maximum of 33 days to correct the
condition that initiated the action. QIS' experience shows that in the vast majority of cases, you will not even
know that this has occurred. If you register with QIS, you will receive multiple warning notices that will allow
you to react to the FMCSA's action in a timely manner.
The most frequent reasons your Authority can be jeopardized are:
A Cancellation of Insurance notice is issued to the FMCSA
Failure to provide the FMCSA with:
Automobile Liability - Form BMC-91/91X
Cargo Insurance (-C authority - Form BMC-34/32)
Broker's Surety Bond - Form BMC-84
Missing Process Agent designation - Form BOC-3
As you see, you may not be in control of the primary reasons why many Motor Carriers and Brokers have lost their
operating authorities. Someone else has the responsibility to either file these forms online or via postal mail.
This person does not know who your are and will not be effected by your loss.
Once your authority is revoked, it can take several days and the payment of reinstatement fees to get the issues
corrected. All the while, your vehicles must be parked, probably with client's cargo on board, going nowhere. This
can be devastating to your company's reputation and could result in the permanent loss of business opportunities.

The following requirements are needed to register:
Your USDOT and MC numbers
Contact Name
Email address that is checked at least once per week**
Your choice of either your Physical or Mailing Address**
Phone and Fax numbers (Cellular Phone Numbers are acceptable)**
Web site if available
payment of an extremely low annual fee
** QIS does not use this registration information for any purpose other than to provide the stated service. Further,
a FREE listing of your company's contact information will be added to QIS' Carriers web page, unless you choose not
to be included.

Once registered and the fee is paid, you will be notified that QIS has initiated the monitoring process for your company.
If for any reason the FMCSA does take action against you, you will receive an immediate notification that includes:
Date of action
Type of action
Cause for action
Insurance,
Process agency, or
Undetermined reason
Contact information for your regional FMCSA field office
